Evan Bash, M.D.
ORTHOPAEDIC ASSOCIATES - Dr. Bash joined the Orthopaedic Associates Division of Premier Orthopaedics in 1989 as a General Orthopedist. He completed his orthopaedic residency from Tufts, New England Medical Center in 1987. Dr. Bash is the football team orthopaedist for the Glen Mills Schools. Dr. Bash also has a special interest in Foot and Ankle Surgery.

Dr. Bash graduated with a BA from Williams College. He received his MD degree from Jefferson Medical College in 1982. He then completed his orthopaedic residency at Tufts-New England Medical Center, where he was Chief Resident in 1987.

Dr. Bash is a resident of Delaware County. He is a Board Certified member of the American Academy of Orthopaedic Surgeons. He is also a member of the Pennsylvania Orthopaedic Society, Philadelphia Orthopaedic Society, Delaware County Medical Society, Pennsylvania Medical Society, and the American Orthopaedic Foot and Ankle Society.

Dr. Bash resides in Wallingford with his wife and four children. He maintains medical staff privileges at Crozer Keystone Health System and Main Line Health System/Riddle Memorial Hospital

Louis J. Ciliberti, DPM, MS
PA ORTHOPAEDIC CENTER - Dr. Ciliberti graduated from the University of Delaware in 1999 with a B.S. in Nutritional Sciences and minors in Biology and Chemistry. He then attended the University of Louisiana at Lafayette and graduated with an M.S. in Human Nutrition in 2002 after completing a graduate assistantship. He maintained a 4.0 GPA and was honored as a "Walk of Honor" recipient.

While attending the University of Louisiana, Dr. Ciliberti was encouraged to pursue a career in Podiatric Medicine and Surgery. He was accepted at the New York College of Podiatric Medicine and graduated in 2008 as a Doctor of Podiatric Medicine. He went on to complete his residency at Texas A & M University Health Sciences Center/ Scott and White Memorial Hospital and the Central Texas Veterans Health Care System in Temple, Texas in 2011.

Dr. Ciliberti had the opportunity to train and work with many leaders in the Podiatric and Orthopaedic community while completing his residency. As a chief resident, he was awarded an AO mini trauma fellowship in Europe. These fellowships are awarded to 8 Podiatric physicians yearly in the United States. Immediately following residency, Dr. Ciliberti completed a one year foot/ankle trauma and ankle replacement fellowship under the guidance of Dr. Jason Miller at the Pennsylvania Orthopaedic Center.

Dr. Ciliberti has a special interest in research and has published multiple papers in peer review journals. He has also presented multiple research posters at conferences in the United States and Europe. He was recently awarded the "Young Scientist Award" by the World Congress on Debates and Consensus in Bone, Muscle and Joint Diseases.

Paul Horenstein, M.D.
SURGICAL ORTHOPEDICS - Dr Horenstein is a general orthopedic surgeon who specializes in foot and ankle surgery and joint replacements.
After receiving his medical degree from Jefferson Medical College, Dr. Horenstein completed his orthopedic residency at Thomas Jefferson University Hospital and a fellowship in foot and ankle surgery at The Massachusetts General Brigham and Women's Hospital.

Board certified by the American Board of Orthopedic Surgery, Dr. Horenstein is affiliated with Roxborough Memorial Hospital, Delaware County Memorial Hospital, Riddle Memorial Hospital, Mercy Fitzgerald Hospital, Springfield Hospital and the Surgery Center of Pennsylvania. His professional memberships include the American Academy of Orthopedic Surgeons, Delaware County Medical Society, Pennsylvania Orthopedic Society and the American Orthopedic Foot and Ankle Society.

Dr. Horenstein has been practicing in the Delaware County and Philadelphia area for ten years. He is a family man who enjoys golf, reading and playing tennis.

Craig Kriza, DPM
ORTHOPAEDIC ASSOCIATES - Dr. Kriza, a past-president of the Pennsylvania Podiatric Medical Association, practices general podiatric care of the foot and ankle, with special interest and concentration in Sports Medicine, diabetic care, and wound management. He joined Premier Orthopaedics in 1995. Dr. Kriza graduated from the Ohio College of Podiatric Medicine followed by a surgical residency in podiatric medicine and a degree from Widener University, School of Law.

Dr. Kriza currently is a consultant/Podiatrist for the Philadelphia Union Soccer Team and Swarthmore College Athletic Department. Also, he is consultant to the executive board of the Pennsylvania Podiatric Medical Association and the executive board of the Goldfarb Foundation.

Dr. Kriza was voted Podiatrist of the Year in 2006 by the Pennsylvania Podiatric Medical Association. He is currently a member of the American Podiatric Medical Association, the American College of Legal Medicine, the American Podiatric Circulatory Society and the Pennsylvania Podiatric Medical Association.

Jason R. Miller, DPM
PA ORTHOPAEDIC CENTER - Dr. Jason R. Miller graduated with honors from Moravian College in 1993 with a B.S. in Pre-Med Biology. He then went on to graduate in 1997 from the Temple University School of Podiatric Medicine as a Doctor of Podiatric Medicine. Dr. Miller completed a 4 year residency at Temple University Hospital where he specialized in all aspects of adult and pediatric reconstructive and traumatic foot and ankle surgery.

Dr. Miller served as Chief Resident and was recognized for assisting in pioneering the foot and ankle program at the 700+ bed Level 1 trauma center in Philadelphia, PA.

Dr. Miller began his career in 2001 teaching in the Dept of Surgery at the Temple University School of Podiatric Medicine where he served as Residency Director of the Temple University Health Systems/Northeastern Hospital podiatric residency program and Chief of the Section. In 2002, Dr. Miller was honored in the Temple University “Gallery of Success”, an annual prestige that is awarded to a recent and past Temple graduate who has excelled in their chosen career. Dr. Miller continues to hold the rank of Associate Professor as an adjunct faculty member in the Dept. of Surgery at TUSPM. In 2008, Dr. Miller was awarded by the Pennsylvania Podiatric Medical Association with the “Rising Star” award.

Dr. Miller has written articles in several peer reviewed journals, written and edited chapters in board review books and lectured both nationally and internationally on a variety of topics related to foot and ankle surgery. Dr. Miller currently serves on the editorial board of The Foot and Ankle Journal Online and serves as a consultant to several pharmaceutical and orthopaedic companies. Dr. Miller is a Diplomate of the American Board of Podiatric Surgery where he is board certified in Reconstructive Rearfoot /Ankle Surgery and Foot Surgery. He is a Founding Fellow of The American Professional Wound Care Association and a Fellow of the American College of Foot and Ankle Surgeons.
